• To register complete this form and hit "Submit" or print this page, fill it out and drop it at Fat Daddy's
• Registration is not complete until your payment is received in full.
• Every regular team player and substitute must sign a player release of liability
• No Dogs are allowed
• No Carry-in items are allowed. This is a bar league, but it is still a bar people! Anyone in violation will be asked to leave
• Leagues Start at $325. League fees will be discounted for prepayments received before 3-28-16 down to $300. Drunken Tourney's $150 and Charity Tourney's Starting at $150 depending on the charity Tournaments: Cash or Check only
• Four-man Leagues start at $250. League fees will be discounted for prepayments received before 3-28-16 down to $225. League Start Dates vary throughout the beginning of April, starting April 10th.
• COED 6'S $325, 4 MAN $250, & FRIDAY NIGHT B/C LEAGUE $175. League fees will be discounted for prepayments received before 3-28-16 down to $150
• Payment for tournaments can ONLY be cash or check!
